About RxTogether

AccessHealth is aware that people in greatest need cannot consistently access affordable medications. Those who are low income, uninsured, and/or have chronic conditions are most severely impacted by lack of access to affordable medications.

Through RxTogether and its partners, valuable resources have been extended to primary care health centers to increase access to 340B and free medication to those patients in need. Improved access to quality Care is accomplished through a wide range of services including complete pharmacy start and management and contract pharmacy services.

An Innovative Approach

RxTogether and its partners have launched an innovative integrated approach to chronic disease and health system transformation. This project builds upon many of the recommendations of national groups such as the American Academy of Family Physicians and the American College of Physicians for a new model of more effective, patient-centered health care.

Health Care Initiatives

In response to the health challenges facing West Virginia, the Network and its members have identified a number of significant health improvement opportunities and have targeted diabetes, cardiovascular risk reduction, depression, weight management and physical activity, asthma, cessation of tobacco use, and oral health as focus areas of its health improvement program, using state of the art technology applications designed to realign the primary care health delivery system to meet the needs of an aging and chronically-ill population. The Network has developed and currently operates a centrally-hosted electronic health information system and an integrated telehealth system as core components of its health improvement platform.
